摘要：目的   研究门诊13C-尿素呼气试验检查患者的临床特征及相关因素，探索临床症状对13C-尿素呼气试验检查的指导作用｡方法  以问卷调查形式，收集2013年3月1日-6月1日门诊行13C-呼气试验检查150例患者的资料进行分析，应用SPSS14.0软件进行数据分析，应用logistic回归筛选检出相关的因素｡结果  共获得有效问卷128份，幽门螺杆菌感染阳性者58例占45.3%；临床症状：报警20例占15.6%，消化不良22例占17.2%，反流18例占14.1%，重叠30例占23.4%，其他症状38例占29.7%；多因素logistic回归分析结果显示，上腹部烧灼感､胸骨后疼痛和便秘与幽门螺杆菌感染密切相关（P <0.05）；饮酒是幽门螺杆菌感染阳性的相关因素（P <0.05）｡结论  基于幽门螺杆菌感染在功能性消化不良、反流性食管炎、便秘等疾病发病机制中的作用，上腹部烧灼感、胸骨后疼痛、便秘症状对于筛选13C-尿素呼气试验检查､提高消化科门诊成本效益具有一定的意义｡

Abstract: OBJECTIVE To study the clinical characteristics and related factors for the outpatient department patients undergoing the 13C-urea breath test so as to explore the role of clinical symptoms in guiding the 13C-urea breath test. METHODS By means of questionnaire survey, the data of 150patients who underwent the 13C- urea breath test in the outpatient department from Mar 1to Jun 1,013 were collected and analyzed, then the statistical analysis of the data was performed with the use of SPSS14.0 software, and the logistic regression analysis was conducted to screen out the related factors. RESULTS Totally 128valid questionnaires have been recovered. The Helicobacter pylori infection occurred in 58 (45.3%) cases. Among the cases of clinical symptoms, there were 20 (15.6%) cases of alarming, 22 (17.2%) cases of dyspepsia, 18 (14.1%) cases of reflux, 30 (23.4%) cases of overlapping, and 38 (29.7%) cases of other symptoms. The result of multivariate logistic regression analysis indicated that the H. pylori infection was related to the epigastic burning, etrosternal pain, and constipation (P<0.05); alcohol intake was the related factor for the H. pylori infection (P<0.05). CONCLUSION As based on the role of H. pylori infection in the pathogenesis of functional dyspepsia, reflux esophagitis, and constipation, the epigastic burning, retrosternal pain, and constipation have certain significance in screening out the 13C-urea breath test and increasing  the cost effectiveness of gastroenterology department.
